Southwark Carers, Lambeth Carers Hub and Link Age Southwark are coming to the surgery with information about the local support available for carers. If you help a relative or friend with a long term condition, or if someone relies on you because of their physical or mental health then this event is for you. Tea and coffee and snacks will be provided.Feel free to drop in any time between 2pm and 3:30pm. If the time isn't convenient then ask at reception any day that week for our new printed resource pack for carers.

Please print off the registration form through this link Registration Form, fill it out and bring it in with you to the practice to register, together with evidence of your address eg tenancy agreement, utility bill, council tax. Please bear in mind that you usually will need to wait two working days to make an appointment whilst we process the registration form.

We don't currently have the facility to register online. Please print off the registration and medical questionnaires and bring them to the practice.
