Accessing Someone Else's Information

Some patients find it helpful for a second person to have access to their online GP record. This is often a parent, family member, next of kin, close friend, carer or someone they trust to act on their behalf. This is called having proxy access. If you’re aged 16 or over, you can ask for someone to be your proxy for any reason. It does not stop you having access to your GP services yourself.

Online Advice

Elm Lodge Surgery has introduced a modern GP appointment system from 2nd September 2026. Completing our consultation form is required before any GP appointment can be booked.

You can fill in the form using the NHS App, through our website, or our reception team can help you. Let us know what's the matter, and one of our GPs will review your request and decide the best way to help.

We will usually be in touch the same day. This could include a telephone or face-to-face appointment, an appointment with another member of our clinical team such as a nurse, physiotherapist or pharmacist, advice without needing to come in, or being directed to another service.

It is easier for us to help you in a timely manner if you can submit your request earlier in the day.

If you cannot complete the form online, please call the surgery and our reception team will help you.

There are no changes to nursing appointments for wound care, vaccinations, ear wax removal, and reviews of long-term conditions such as diabetes, asthma, or COPD. Call as usual for these.

If you need immediate medical attention, please call 111 or go to A&E.
