Information and Privacy Notice

Information Notice

Elm Lodge Surgery collects and holds data about our patients for the purpose of providing safe and effective healthcare. We recognise that our patients entrust sensitive and personal information to us and we have a responsibility to keep it accurate and secure.

If you want to know how we keep your data confidential and secure please read our information notice (pdf file).

Information Notice for Patients

If you want more information about how your health information is used across the whole NHS please follow this link to the South East London ICS Website.

We publish our  Data Security and Protection Policies in order to meet the transparency requirement of GDPR.

Telephone Call Recording

The surgery records all inbound and outbound phone calls for staff training and audit purposes.

National Data Opt-Out

The NHS uses patient data to provide healthcare and also to research, plan and improve services. Find out more about how your data is used, and how to opt-out of data sharing here.

Subject Access Requests

A request by a patient, or a request by a third party who has been authorised by the patient, for access under the GDPR (and DPA 2018) is called a Subject Access Request (SAR). If you want to see your health records, or wish a copy, please write or email the practice or contact the practice for assistance. Contact will, subsequently, be made by the practice to arrange a time for you to come in and collect or read them. You don’t have to give a reason for wanting to see your records and there is no charge for this service. You will however be required to produce proof of identity before being allowed to read them.

The practice has up to 28 days to respond to your request. If additional information is needed before copies can be supplied, the 28-day time limit will begin as soon as the additional information has been received.

The 28 day time limit can be extended for two months for complex or numerous requests where the data controller (usually your practice) needs more time to collate and supply the data. You will be informed about this within 28 days and provided with an explanation of why the extension is necessary.

Inaccurate Data

If there is inaccurate factual data in your medical records please write to or email the practice.

Your Contact Preferences

The practice may contact you by SMS, telephone, letter and email in order to make appointments, remind you of booked appointments, or communicate about other matters related to your care at the surgery. Please write to us, or speak to the reception team if you would like to express a preference or object to being contacted in these ways.
