Anyone in England can register with their local GP surgery.
You do not need proof of address or immigration status, ID or an NHS number.

Completing a Registration Form
Our reception team can help you register with us. Please come in between 9am and 6pm and bring:
- The name and address of your previous NHS GP, or the date you have entered the country if you don't have a previous GP.
- Details of your current and previous address.
- Photo ID and proof of address if you have these. If you don't, we can still register you but it may take longer to trace your previous notes and/or make a referral.
- When registering infants please bring their red book.
If you do not speak English or find it hard to complete forms for any reason please let us know so that we can help with the paperwork.
If you prefer to register without coming into the surgery please complete a GSM1 form and either an adult new patient questionnaire or child new patient questionnaire. These can be scanned and emailed to SOUCCG.firstname.lastname@example.org or posted to us.
